RELEASE OF LIABILITY, WAIVER OF CLAIMS AND INDEMNITY AGREEMENT

1. Acknowledgment and Assumption of Risk
I acknowledge that participation in tennis and related activities at Urban Tennis involves inherent risks that may result in injury.
These risks include, but are not limited to:

• Being struck by tennis balls, rackets, or other equipment

• Slipping, tripping, or falling on courts or surrounding areas

• Physical exertion resulting in injury, illness, or fatigue

• Collisions with other participants

• Equipment failure or improper use of facilities

I understand that these risks may result in serious injury, permanent disability, or death.
By participating in activities at Urban Tennis, I voluntarily accept and assume all risks, both known and unknown.

2. Release of Liability
In consideration of being permitted to use the facilities and services of Urban Tennis, I hereby release, waive, and discharge Urban Tennis, its owners, directors, employees, coaches, contractors, volunteers, and affiliates from any and all claims, liabilities, demands, damages, or causes of action arising from participation in tennis or related activities.
This release includes claims resulting from negligence to the fullest extent permitted by law.

3. Indemnification
I agree to indemnify and hold harmless Urban Tennis from any claims, damages, losses, liabilities, costs, or legal expenses arising from my actions while using the facilities or participating in activities.

4. Health and Physical Condition
I confirm that:

• I am physically able to participate in tennis activities

• I have no medical conditions that would prevent safe participation

• I will immediately stop participating if I experience pain, dizziness, or discomfort
   

5. Facility Rules and Conduct
I agree to follow all rules and policies of Urban Tennis, including:

• Respecting court reservation times

• Wearing appropriate non-marking tennis shoes

• Using equipment safely and responsibly

• Following staff instructions

Urban Tennis reserves the right to suspend or terminate facility access for violations of club rules.

6. Personal Property
Urban Tennis is not responsible for lost, stolen, or damaged personal belongings brought onto the premises.

7. Emergency Medical Care
In the event of injury or medical emergency, I authorize Urban Tennis staff to obtain emergency medical assistance on my behalf if necessary.
I acknowledge that I am responsible for any resulting medical costs.

8. Photography and Media
I grant Urban Tennis permission to use photographs or videos taken during club activities for marketing, promotional materials, and social media.
If I do not wish to be photographed or recorded, I will notify staff in advance.

9. Participation of Minors
If this agreement is signed on behalf of a minor, I confirm that I am the minor’s parent or legal guardian.
I consent to the minor’s participation in activities at Urban Tennis and agree to all terms of this waiver on the minor’s behalf.

10. Governing Law
This agreement shall be governed by the laws of the Province of British Columbia, Canada.

11. Club Rules
Urban Tennis facility rules include:

• Non-marking tennis shoes required

• Courts must be vacated when reservation time ends

• No outside coaching without permission

• Management reserves the right to cancel reservations or suspend access if necessary
